How can you join the scheme?
There are 3 ways to be referred onto the 12-Week Active Heart scheme:
- Hospital Referral
- Individuals must have completed their Phase 3 Cardiac Rehabilitation programme at a hospital within the catchment area (Leicester City Hospitals, Queens Hospital in Burton Upon Trent).
- Referral from your GP
If you have been diagnosed with heart disease contact your GP/Practise Nurse who can refer you on to our scheme.
Active Hearts gym sessions
The gym sessions last for 12 weeks and are specifically designed for clients with moderate to low risk and are held Hermitage, Hood Park and Measham Leisure Centres.

Tuesday 2pm-3pm Cardaic Gym Hood Park Lesisure Centre £3.50ps
Thursday 1.30pm-2.30pm Cardiac Rehabilitiation Hermitage Leisure Centre £3.50ps
Friday 1pm-2pm Cardiac Condition Hood Park Leisure Centre £3.50ps

Participants are offered safe and effective exercise
- Under the guidance of a qualified B.A.C.P.R Exercise professional (British Association of Cardiac Prevention and Rehabilitation)
- At a venue in North West Leicestershire District
- At a reduced rate
Active Hearts is a cardiac rehabilitation scheme run by North West Leicestershire District Council in partnership with Leicester-Shire and Rutland Sport (LRS), University Hospitals and local GP Surgeries.
