Complete this form to make sure that your event is safe and not disrupted by services like gas, water and electricity, road works, telecommunications etc.  You must complete this form before your event takes place to ensure the lowest possible disruption to your event.  If you do not complete this form, actions by other organisations might seriously impact your event.
